Tämän opinnäytetyön tarkoituksena on selvittää työntekijöiden sekä mielenterveys- ja päihdeasiakkaiden kohtaamistilanteita kotihoidossa työturvallisuuden näkökulmasta kuvailevan kirjallisuuskatsauksen avulla. Opinnäytetyön tavoitteena on tarkastella ja käydä kriittisesti läpi jo olemassa olevia tutkimuksia ja antaa kotihoidon työntekijöille näyttöön perustuvaa ja ajankohtaista tietoa työntekijöiden työturvallisuudesta mielenterveys- ja päihdeasiakkaiden kohtaamistilanteissa. Opinnäytetyö päädyttiin toteuttamaan kuvailevana kirjallisuuskatsauksena. Kuvailevan kirjallisuuskatsauksen teossa ei ole tiukkoja ja tarkkoja sääntöjä. Tämä mahdollistaa laajan aineiston käytön ilman metodisia sääntöjä. Opinnäytetyön aineistoiksi valikoitui tutkimuksia, raportteja, pro-gradu tutkielma sekä opas. Opinnäytetyössä käytettiin yhteensä kuutta tutkimusta. Aineistoja läpi käydessä voidaan yhteenvetona todeta, että kotihoidossa hoitajiin kohdistuu väkivaltaa, yleensä väkivalta on sanallista. Mielenterveys- ja päihdeongelmat ovat kasvava ongelma kotihoidossa. Kyseiset asiakkaat aiheuttavat kotihoidontyöntekijöille erinäisiä vaaratilanteita ja työturvallisuusuhkia. Tuloksista käy ilmi, että hoitotyöntekijät kokevat aggressiivisen asiakkaan kohtaamisen haastavana sekä valmiudet kohtaamiseen puutteellisena.

Tavoitteenani oli tutkia lasten aggressiivisen käytöksen ilmentymistä varhaiskasvatuksessa, aggressiivisen käytöksen syitä ja sitä, miten varhaiskasvattajien tulisi puuttua aggressiiviseen käytökseen varhaiskasvatuksessa. Halusin tutkia nimenomaan aggressiivisuutta, joka ilmenee sekä fyysisenä että sanallisena aggressiivisuutena. Keskityin työssäni varhaiskasvatuksen kasvuympäristöön. Opinnäytetyöni on toteutettu laadullisena tutkimuksena. Keräsin tietoa avoimien kysymyksien avulla varhaiskasvatuksenopettajilta kyselylomakkeen muodossa. Halusin tietää ovatko varhaiskasvattajat kohdanneet aggressiivista käytöstä, miten aggressiivisuus ilmenee ja miten siihen tulisi puuttua. Myös syyt aggressiivisen käytöksen takana olivat isossa osassa. Opinnäytetyöni teoriaosuudessa käsittelin aggressiivisuutta käsitteenä, aggressiivisen käytöksen syitä, biologisia tekijöitä sekä ympäristöllisiä tekijöitä. Aggressiivisuuden sukupuolieroja käsitellään myös teoria osuudessa. Teoriaosuudessa puhutaan myös aggression portaista sekä varhaisen vuorovaikutuksen merkitysestä. Tutkimus tulokset kertovat, lasten aggressiivisuus on lisääntyvä ilmiö, josta ei puhuta tarpeeksi. Lähes jokainen varhaiskasvattaja kohtaa aggressiivista käyttäytymistä työssään.

The purpose of this thesis was to describe how parents have experienced the aggressiveness of their child or adolescent with special needs, what kind of negative emotions the parents experienced and how these emotions impacted their resources. The aim was to provide information and increase the professionals' understanding of the issue so that they could be more understanding when encountering parents and provide individualised support to families right from the beginning of the care relationship. The research method was qualitative. The data for this study was collected through thematic interviews. The study was part of the development project "When a child is violent" (Kun lapsi satuttaa) in the Pirkanmaa coordinated by Service Foundation for People with an Intellectual Disability. The target group of the thesis consisted of families who had participated in the development project and one family from outside the project. Four parents who had experienced violence from their children or adolescents were interviewed for the thesis. The interviews were conducted remotely due to the prevailing COVID-19 situation. The research data was analysed using a data-driven approach. It became apparent in the study that the forms of violence experienced by parents ranged from verbal threats all the way to physical aggression. The thesis revealed that parents experienced several different negative emotions when confronted with violence from their children or adolescents. In total, twenty-two different negative emotions. Almost without exception, violence from a child or adolescent causes parents to experience strong feelings of shame, fear, sadness and anger. The parents perceived the impacts of negative emotions as manifold and resource-consuming. The most important area for development that emerged from the study was the understanding of the impact of negative emotions experienced by parents on the part of professionals, and thus the building of a more empowering encounter in the care relationship. Further research could be conducted on how the professionals who meet families feel in the situations where they encounter families with a child or adolescent behaving aggressively towards their parents, and how combining these emotional backgrounds would open up the most fruitful basis for care work. The thesis will enable the client to bring the touching stories and voices of families to the forefront and increase the debate around the topic.

Tässä työssä laadittiin raportti siitä, miten Satakunnan sairaanhoitopiirin psykiatrian tulosalueen osastotyötä tekevälle henkilökunnalle suunnattu Aggression hoidollinen hallinta-koulutus toteutettiin. Tämän työn toiminnallisen osion tuotos oli Satakunnan sairaanhoitopiirin Hiisinettiin tuotetut AHHA/TUHO-koulutusprojektin intranetsivut. Sivut sisältävät tietoa Aggression hoidollinen hallinta- ja Turvallinen hoitotyökoulutuksista. Aggression hoidollinen hallinta-koulutus perustuu Englannissa, Birminghamin Reaside klinikassa ja Keelen yliopistossa Staffordsherissa kehitettyyn MAPA-menetelmään. Management of Actual or Potential Aggression eli MAPA tarjoaa koulutusta väkivallan ennaltaehkäisyyn ja turvalliseen hoitoon hoitoalalla. Tampereen yliopistollisen sairaalan psykiatrian tulosyksikkö on kehittänyt MAPA:sta Suomen hoitolaitoksiin sovelletun mallin, Aggression Hoidollinen Hallinta eli AHHA. Keskeistä menetelmässä on väkivaltatilanteiden ennakointi ja välttäminen hoidollisen vuorovaikutuksen ja toiminnallisuuden keinoin. Mikäli väkivaltaisuuden suuntaan kehittyvän käyttäytymisen rauhoittaminen ei muilla keinoin onnistu, turvaudutaan tarvittaessa viimeisenä keinona potilaan fyysiseen kiinnipitämiseen. Koulutuksen kohderyhmänä ovat olleet Satakunnan sairaanhoitopiirin psykiatrian tulosalueen koko henkilökunta ja heistä valtaosa, yli 400 henkilöä, on koulutettu AHHAmallin mukaan. Raporttiosiossa selvitettiin, miten koulutus suunniteltiin ja toteutettiin, myös Satakunnan sairaanhoitopiirin AHHA-kouluttajien suorittaman kyselyn tulokset esitettiin raportissa. Opinnäytetyön toiminnallisessa osuudessa tuotin Satakunnan sairaanhoitopiirin Hiisinettiin AHHA-koulutuksista intranetsivut. Sivusto sisältää tietoa koulutuksien peruslähtökohdista ja sisällöstä. Intranetsivut toimivat tiedonlähteenä uusille koulutettaville ja muistin virkistyksenä henkilöille, jotka ovat jo koulutuksen käyneet.

The topic of this bachelor’s thesis arose at Familar Säikkä's internal training regarding young people's violent behaviour. The work was done to make a guide about aggression education in support of guidance and education work in Familar Säikkä. The goal was to improve my own know-how and skills and the competence of the whole working community, to use different methods and tools and to unify these practices, and there-by support the positive growth and development of children and to prevent aggressive and violent behaviour. The theory base of the guide was created through a systematic literature search in several stages, as well as using expertise, methods and tools already existing at Familar Säikkä. The guide presented the background of aggression, risk factors and things that support children’s normal and positive aggression development. Methods and tools of aggression education were presented both as a list and with examples. In the evaluation of the guide, the perspective of the issues raised and their arguments and the necessity of the topic was considered important. Aggression education was seen to be essential part of the guidance and upbringing of a child. The role of the educator and control of one's own emotions was also seen as important. The guide will be used to familiarize existing and new employees of Familar Säikkä and will also be used in marketing in the future. The development of the guide will continue as part of the development of the guidance and education work.

Nursing professions are at risk of labour violence. In the health and social care sector, employees are in close contact with customers, patients and their relatives. This allows for situations where the risk of being subjected to violent behaviour or experiencing threatening behavior increases. Violence has a significant impact on the well-being of the care worker, staying in work and treating the patient. Physical and verbal violence against a care worker is linked to the stress experienced by the care worker. The purpose of this thesis was to use the survey to determine the number and quality of threatening and aggressive situations experienced by nurses and doctors and to gain an understanding of the usefulness of the training provided on the subject. The aim was to increase the knowledge and capacity of the medical centre's nursing staff in relation to occupational safety through training, and to provide information on the occupational safety experienced by the staff in the work community. Development research and the related survey were used as the development method for the thesis. As an intervention in the thesis, training and the related checklist were used. Education is an inclusive and practical method that brought as many members of the work community as possible into the thesis. The cheklist creates an opportunity for further discussions by the staff. The results of the survey showed that the training and checklist provided to staff was found useful. The results supported the results of the Literature Review, where the most significant way of reducing violence experienced by staff in the workplace was the training offered to nursing staff. Nearly one-half of the respondents surveyed had encountered threats and violence situations in their work that had caused the respondents anxiety and stress.
